Medical Insurance

Stark County has selected Sanford Health to provide health insurance benefits to our employees.  Stark County offers the choice of the Signature PPO Plan or a High Deductible Health Plan.

Sanford Health Signature PPO Plan

• Stark County pays 90% of the single premium and 80% of the premium when adding children, spouse, or family
• $500 individual Deductible, $1,000 family Deductible
• $2,500 out-of-pocket maximum for individual, $5,000 family.
• $30 copay for provider's office visit
• $150 copay for emergency room visit
• 20% coinsurance for diagnostic services, inpatient stay, outpatient surgery
• $0-5 copay for Generic Drugs; $20 copay for Preferred Brand Drugs; $35 copay for Non-Preferred Brand Drugs.

Sanford Health High Deductible Health Plan (HDHP)

• Stark County pays 100% of the premium for any tier
• $3,500 individual Deductible, $7,000 family Deductible
• $3,500 out-of-pocket maximum for individual, $7,000 family
• $0 co-insurance after Deductible is met
• $5 co-pay for Preventative Drugs and all other pharmacy goes towards Deductible
