What You Need to Apply for a Passport
- Scheduled appointment to submit your application.
- Each applicant present with their application.
- Application filled out in black ink only.
- Valid ID choose from the following:
- One Primary ID:
- ND Driver's License (applicant must have had a ND license for at least 6 months)
- Valid or expired U.S. passport book or card
- Government employee ID (city, county, state, or federal)
- U.S. military or military dependent ID
- Current (valid) foreign passport
- Mexican Consular ID
- U.S. Permanent Resident Card (Green Card)
- Enhanced Tribal Cards and Native American tribal photo IDs
- If the applicant does not have a primary ID, they will need either 2 photo IDs or 1 photo ID and 2 IDs with just the applicant's name, such as:
- Out-of-state driver's license
- Learner's or temporary driver's permit
- State issued ID
- Temporary driver's license
- Social Security card
- Vehicle registration
- Employee work ID
- Student ID
- Medicare or other health card
- Expired driver's license
- Certified Copy of Birth Certificate, Naturalization document, or other proof of citizenship.
- Passport photos taken within 6 months.
- One check, money order, or cashier's check PER APPLICATION made payable to the U.S. Department of State.
- Cash or one check, money order, or cashier's check made payable to the Stark County Recorder. This fee can be combined for multiple applicants.
