Minor Passport Application
ALL APPLICANTS (BOTH ADULTS AND MINORS) WILL NEED ITEMS LISTED IN WHAT YOU NEED TO APPLY FOR A PASSPORT SECTION
PARENTS' ID WILL BE USED FOR IDENTIFICATION OF MINORS UNDER 16
Children 15 and Younger
- Each child applying for a passport must be present with both parents at time of application.
- One parent present is acceptable if:
- Only said parent is listed on the birth certificate.
- A completed DS-3053 consent form and front and back copy of other parent's ID is submitted with the application.
- A certified death certificate for the other parent is submitted with the application.
- A certified judgement or court document stating full legal custody and/or right to apply for the child's passport is submitted with the application.
- Parents will need to provide Valid ID as listed above (What You Need...).

If the minor has previously had a passport, the expired passport must be submitted along with the certified copy of the birth certificate.
Children 16-17
- One parent present with application, or proof of parental consent.
- Applicant must provide valid ID if available or if applying only with proof of parental consent.
- Parental Consent can be shown by:
- Parent Applies with You: One of your legal parents or guardians applies with you and signs Form DS-11. The parent or guardian will need a valid ID.
- Parent Signs a Note: Submit a signed note from your legal parent or guardian. Bring a photocopy of that parent or guardian’s ID.
- Parent Pays Fees: Submit a check or money order with your legal parent or guardian’s name.
