1. Scheduled appointment to submit your application.
2. Each applicant present with their application.
3. Application filled out in black ink only.
4. Valid ID choose from the following:
• One Primary ID:
• ND Driver's License (applicant must have had a ND license for at least 6 months)
• Valid or expired U.S. passport book or card 

• Government employee ID (city, county, state, or federal)
• U.S. military or military dependent ID
• Current (valid) foreign passport
• Mexican Consular ID
• U.S. Permanent Resident Card (Green Card)
• Enhanced Tribal Cards and Native American tribal photo IDs

• If the applicant does not have a primary ID, they will need either 2 photo IDs or 1 photo ID and 2 IDs with just the applicant's name, such as:
• Out-of-state driver's license
• Learner's or temporary driver's permit 

• State issued ID
• Temporary driver's license
• Social Security card
• Vehicle registration
• Employee work ID
• Student ID
• Medicare or other health card
• Expired driver's license 

1. Certified Copy of Birth Certificate, Naturalization document, or other proof of citizenship.
2. Passport photos taken within 6 months.
3. One check, money order, or cashier's check PER APPLICATION made payable to the U.S. Department of State.
4. Cash or one check, money order, or cashier's check made payable to the Stark County Recorder. This fee can be combined for multiple applicants.

• Each child applying for a passport must be present with both parents at time of application.
• One parent present is acceptable if:
• Only said parent is listed on the birth certificate.
• A completed DS-3053 consent form and front and back copy of other parent's ID is submitted with the application.
• A certified death certificate for the other parent is submitted with the application.
• A certified judgement or court document stating full legal custody and/or right to apply for the child's passport is submitted with the application.
• Parents will need to provide Valid ID as listed above (What You Need...).

• If the minor has previously had a passport, the expired passport must be submitted along with the certified copy of the birth certificate.

Children 16-17

• One parent present with application, or proof parental consent.
• Applicant must provide valid ID if available or if applying only with proof of parental consent.

The Recorder's Office can review and assist in assembling renewal applications. Please schedule an appointment or stop in the office.

Applicants are eligible to renew passports by mail if their most recent passport:

• Can be submitted with the application,
• Is not damaged (other than normal wear and tear),
• Has never been reported lost or stolen,
• Was issued within the last 15 years,
• Was issued when the applicant was age 16 or older, and
• Was issued in their current name or the applicant must provide a document like a certified copy of a marriage certificate or divorce decree to show a name change.

To renew a passport:

• Complete the DS-82 Passport Renewal Application in black ink only.
• Attach a new passport photo to the application as indicated on the form.
• Submit the current passport with the application.
• Attach 1 check or money order paid to the U.S. Department of State to the application. Please include the applicant's full name and date of birth on the check or money order.
  

Submit the application, passport, and payment through the mail. It is recommended to use a USPS Priority or Priority Express envelope to obtain a tracking number.
